Nursing:
Registered nurses assess patients and a treatment plan to assure quality care from a patient’s admission to their discharge. This care plan assists patients and families in maintaining an optimum level of functioning and independence.
Licensed Practical Nurses:
Work as team members functioning under the registered nurse to assist in implementing the plan of care.
Home Health Aides:
Certified nursing assistants provide personal care and assistance with bathing, dressing, feeding, grooming and light housekeeping tasks while maintaining a safe environment for the patient.
Physical Therapy:
Registered physical therapists create a home exercise program tailored to the unique needs of each patient. The therapy treatment plan may included balance exercises, gait training, range of motion, manual therapy techniques, stair training and specific exercises to restore the patient to maximal functioning. Ultrasound treatment, chest physical therapy and cardiac rehabilitation are also available.
Occupational Therapy:
Registered occupational therapists provide assessment and services to increase the patients’ ability to perform daily living tasks such as personal hygiene, dressing, grooming and meal preparation. Specific adaptive equipment is incorporated into care as needed and customized equipment such as specialty wheelchairs are ordered as appropriate.
Speech Language Pathology:
Licensed speech therapists assess and treat speech, language, cognitive and swallowing disorders. A strong focus is placed on the goal of improving communication techniques.
Medical Social Services:
Experienced social workers will evaluate the need for community resources and make referrals as appropriate. Social work services are also available to assist in long term planning, help patients and families adjust to the needed lifestyle changes related to the patient’s illness or disability and offer end-of-life support.
